The difference between President and Prime Minister is based on the structure of the British government. The Queen is the head of state and generally serves in a ceremonial and diplomatic role. The Prime Minister is the head of the legislature and also the head of the various government ministries. In countries with this form of government, there is no seperate legislative and executive branches as in the United States. In countries with this system that have a constitutional monarchy the king or queen serves as the head of state; in governements where there is no monarch, there is usually a president instead.
In American politics, the Presisdent is both head of state and head of government, so he serves the role a monarch would in a constitutional monarchy, but he also is in charge of the various departments of the Federal government, which serve the same purpose as the various ministries in other governments. Unlike a Prime Minister, he is independant of Congress, which is supposed to balance power between the two branches.

EDIT: Also, I might be wrong, but I'm pretty sure the French government has a President and a Prime Minister too. But the reason why Russia implemented it fairly recently? Leish summed it up pretty well. Medvedev is just a Putin-puppet.
