I need a Stat Chart for Monsters

I'm writing information on my Characters and Monsters for my upcoming game Siv Terra: Cataclysms. I need a chart for all of the Monsters editing sections so that I can fill them in for my creatures. Please someone take the time to help me? Let me rephrase, There are 4 section in the monster editting process right? I only remember the first three and I don't remember if there's one after the skills page or one before it. I could use a chart for the skills editor as well please..... I'm not asking for you to fill it in, I'm just asking for a blank skills editor chart pleaz.

....Why don't you just boot up RPGM3 and look for youself?
