I like to make a variety of different types of items (some which must be found and cannot be bought in stores). Also, treasure items are a must, in my opinion.
I have everything from "Poison Bye Bye" to "Da Bomb" as items in my game.
I love giving them new original* names. It makes them unique.
*well, after all nothing is totally original, right.
These are just the ones I can remember off the top of my head. Many are multi-use items.

There's a lot of fun stuff you can do when your game world doesn't take itself too seriously.
Now for weapons, I didn't want to be outlandish, so I stuck with a mixture of the usual stuff and some exotic weapons from abroad. Here are some nice dagger names...
Katar - A dagger from India. Has a handle for the hand to grip, and the blade juts straight out from the knuckles. Also called a "punch dagger".
Kukri - A Nepalese dagger. Has a mean curve with a single edge on the inside.
Kris Blade - Wavy assassin's knife.
Khopesh - Egyptian sword, actually, but there's a dagger model that kinda sorta looks like one. A khopesh has a blade that's kinda shaped like a question mark. Got a crescent curve on it for slashing.
I was thinking of making a thread like this, except with monsters.
For basic "items" I have:
Potion, Potion High, and Potion Ex (HP recovery, two uses) Life Scroll (heals all party members, one use) Pot Roast (Massive healing to one member, two uses) Life Stone (Heals all members, six uses,)
MP Restore, MP High, MP Ex, MP Heal Scroll, White Cake... All the same as above, except with MP.
Bomb Bag, Bomb Pack (damages enemy, Bag has two uses, Pack has six, same damage for each) Death Scroll MP Drain Scroll, Pumpkin Pie, Death Stone... etc
Bright Energy (Raises encounter rate) Dark Energy (lowers encounter rate)
For Equipment:
Warrior Ring (increases STR and DEF) Mage Ring (increases MAG and MDF) Eagle Ring (increases STR, MAG and Agility, and mkaes one Swift) Magnetic Ring (increases DEF, MDF, and Agility, and gives Enrage) Runed Ring (increases all stats by a small amount and restores MP) Blood Ring (same as above, except it restores HP) Scale Helm (improves STR, MAG, DEF, and MDF, only usable by certain characters) Parasite Mask (improves all stats)
I usually try to stick with the "standards" for items (Med Herb, Potion, etc.) but with weapons its really hard for me to call something a "Short Sword" or a "Hand Axe" because it seems so ...dull. I mean, okay, sure, if something is sold everywhere then it really isn't all that special, right? But if you went and took the time to name the shopkeeper/blacksmith/whatever then wouldn't you think that "Tim" would make different things than "Brandi"???
I've been toying with the same game for several years now (EXTREMLY Epic) but I've basically decided that each character will get 10 weapons (each unique) and each weapon will have (at least) 3 variations (junk, standard, exceptional) that you come across. Obviously, the junk will be sold in stores, but if the player explores REAL hard (or fights a lot of battles...) they can get the high-end stuff.
FUN names that I came up with:
Maidurango
Kroovy
Geggy Tah
Lich Mark
Girder
Skullaxe (one of my less inventive names)
Mirado
Pained Fury
Powertrip
Wind Fall
Iron Dragoon
...the list goes on. Basically I need to be in the right kind of mood to think up good names for things. If the character is dark and brooding I find I'm happier with the equipment names if I share the same motivation. Aside from that, I usually just try to envision what I want the character to be hacking a bloody swath with, and then try to think of a name that fits: like naming a puppy!
